Long
Term Application
Students from within
Florida (or Grand Bahama Island) must use this form to apply for the year-long
exchange program. Use Adobe Acrobat Reader software to open, complete, and
print the form (you'll be able to save your work with Adobe Reader Version
8 or newer).

Students should use this form if applying for the short-term
(summer) exchange program. Use Adobe Acrobat Reader software to open,

Short-term exchanges are not arranged or managed by the Rotary Youth
Exchange Florida Multi-District organization, but rather by the
individual member districts. Contact your local
district Youth Exchange
Chairperson to determine if the Short-term program is offered in your
area, and, if so, the requirements, deadlines, and procedures that you
must follow.

for Inbound Students

Travel
Arrangement Form
All inbound students are required to complete and submit
this form prior to arrival, to provide the details of their flights. |
|
Inbound students
coming to Florida must bring or mail this form, signed by their
natural parents, to authorize their travel while here on exchange.
|
|
All
inbound students are required to submit this form, signed by the student
and his/her parents, before the beginning of the exchange. Please read
carefully the rules that are specific to our program, as you will be
expected to abide by them.
|
|
This form must be completed and signed by a doctor in the
home country, to provide a complete list of the student's immunizations
in a format that will meet Florida school standards. Even if the
original YE application had some or all of this information, this form
is required.

This
document provides detailed information on the procedures to be
followed by long-term inbound students in order to obtain their J-1
student visas.

Government on what inbound exchange students should expect upon arrival
at a US airport.

All inbound students should read this letter from the US
Department of State, as it contains important information for them.

Inbound students are required to submit this form when they
change host families. It must be filed within 5 days of the date of the
move.
|
|
All inbound students are required to complete
and submit this form on October 15, January 15, and April 15, to provide
RYE-Florida and the host district with a record of the exchange.
|
|
Any inbound student who wishes to travel outside the
district for more than 24 hours is required to complete and submit
this form to request district approval of the trip. Travel plans
should not be confirmed until district approval is granted.

Other

All host
families are required to complete this form and submit it to the host
Rotary Club and to RYE-Florida. This is a requirement of the US
Department of State for all exchange programs.
