Long Term Application

Students from within Florida (or Grand Bahama Island) must use this form to apply for the year-long exchange program. Use Adobe Acrobat Reader software to open, complete, and print the form (you'll be able to save your work with Adobe Reader Version 8 or newer).

Short Term Application

Students should use this form if applying for the short-term (summer) exchange program. Use Adobe Acrobat Reader software to open, complete, and print the form. Instructions are included. NOTE: Short-term exchanges are not arranged or managed by the Rotary Youth Exchange Florida Multi-District organization, but rather by the individual member districts. Contact your local district Youth Exchange Chairperson to determine if the Short-term program is offered in your area, and, if so, the requirements, deadlines, and procedures that you must follow.

Forms for Inbound Students

Travel Arrangement Form
All inbound students are required to complete and submit this form prior to arrival, to provide the details of their flights. 

Travel Authorization

Inbound students coming to Florida must bring or mail this form, signed by their natural parents, to authorize their travel while here on exchange.

RYE Florida Rules and Regulations

All inbound students are required to submit this form, signed by the student and his/her parents, before the beginning of the exchange. Please read carefully the rules that are specific to our program, as you will be expected to abide by them.

Certificate of Immunization

This form must be completed and signed by a doctor in the home country, to provide a complete list of the student's immunizations in a format that will meet Florida school standards. Even if the original YE application had some or all of this information, this form is required.

Visa Instructions for Inbound Students

This document provides detailed information on the procedures to be followed by long-term inbound students in order to obtain their J-1 student visas.

US Entry Procedures

This document contains information and advice from the US Government on what inbound exchange students should expect upon arrival at a US airport.

This publication provides important information for inbound exchange students, and serves as a reference before and during the exchange year.

Department of State Letter to Inbound Students

All inbound students should read this letter from the US Department of State, as it contains important information for them.

Host Family Change Report

Inbound students are required to submit this form when they change host families. It must be filed within 5 days of the date of the move.

Quarterly Report Form

All inbound students are required to complete and submit this form on October 15, January 15, and April 15, to provide RYE-Florida and the host district with a record of the exchange.

Inbound Travel Request

Any inbound student who wishes to travel outside the district for more than 24 hours is required to complete and submit this form to request district approval of the trip. Travel plans should not be confirmed until district approval is granted.

Other Forms and Documents

Host Family Application

All host families are required to complete this form and submit it to the host Rotary Club and to RYE-Florida. This is a requirement of the US Department of State for all exchange programs.

Department of State Letter to Host Families

All host families should read this letter from the US Department of State, as it contains important information for them.

Forms for Club and District Youth Exchange Officers

Youth Volunteer Application

All volunteers working with the Rotary Youth Exchange program are required to complete, print, sign, and submit this form to RYE-Florida.

Host Student Information Summary
This form should be attached by District YE Chairs to the completed Guarantee Forms when they are returned to RYE-Florida.

Fillable Guarantee Form
This form should be used by YEOs of host districts and/or clubs to complete all the required information on the Guarantee Form, prior to obtaining the endorsements of the host club and school.

Inbound Student Airport Check-in Form

Cash Receipt
This form should be completed by a District Youth Exchange Committee representative who meets the inbound student at the airport, or within a few days of arrival. Copies are sent to he RYE-Florida chair and the host district YE chair.

When collecting the $300 emergency fund, make sure to fill out and give the student a cash receipt (page 2)

Counselor's Monthly Report Form
This form should be completed on a monthly basis, and submitted to the RYE-Florida chair and the host district YE chair. The form is designed to be saved by the counselor, updated monthly, and e-mailed as an attachment. Please send the form to Lawrene Bishop lawrene@ryeflorida.org.

Second home visit to host family
This form is used to report the suitability of the host family to continue hosting an exchange student.  This visit must be conducted in-person, by a someone other than the individual that secured the host family originally.  If second visit is conducted by an individual cleared and has completed the DOS testing, then visit may also count as a monthly contact. This visit must be completed within 60 days of the students arrival in the home and must be completed each time a student changes Host Families.
